Clinical Documentation Specialist. Full-time Position. Shared position with Patient Business Services and Medical Records. Primary work schedule is 8:00 a.m. to 5:00 p.m. on Mondays through Fridays; however, our incumbent should be flexible to work some evening and weekend hours when business needs necessitate.
Our Clinical Documentation Specialist will facilitate improvement in the overall quality, completeness and accuracy of medical record documentation. She/he will obtain appropriate clinical documentation prior to patients' discharge (concurrent review) through extensive interaction with physicians, nursing staff, Case Management staff, other patient caregivers and Medical Records coding staff to ensure the clinical documentation reflects the severity of illness and level of service rendered to patients. Our Clinical Documentation Specialist ensures that documentation is complete and accurate to obtain appropriate reimbursement. She/he educates all members of the patient care team on documentation guidelines on an ongoing basis to capture best practice indicators to support CMS and Joint Commission requirements.
Minimum qualifications include: Applicants should possess one of the following degrees: Associate of Science Degree in Nursing, Bachelor of Science Degree in Nursing, Associate of Science Degree in Health Information Technology, or Bachelor of Science Degree in Health Information Administration. Nursing graduates must have a current North Carolina Registered Nurse license, and Health Information Technology graduates should have their requisite credentials (RHIT, RHIA or CCS). Applicants should have a minimum of three to five years of clinical experience. Additionally, applicants should have a working knowledge of utilization/case management and of CPT, ICD-9-CM classification system/coding guidelines. Knowledge of DRG and APC reimbursement systems is also required. Applicants should be proficient with basic office equipment, including PC and encoder and must be able to interact effectively with physicians and colleagues.

Speech Language Pathologist(s) - PRN Opportunities. We're recruiting for PRN Speech Language Pathologists to augment our full-time staff. We're offering flexible scheduling to allow incumbents to work various schedules anytime between the hours of 8:00 a.m. to 5:00 p.m. on Mondays through Fridays.
The Speech Language Pathologist is responsible for patient evaluations and/or treatments of any person when so ordered by a Physician. He/she is responsible and accountable for providing direct, age appropriate patient care for the Speech Language patient. The Speech Language Pathologist is responsible for the performance of specialized tests and treatments as well as teaching procedures to the patient, family and other health personnel.
Minimum qualifications include: Graduate of an accredited curriculum in Speech Language Pathology with Master's Degree. Licensed to practice Speech Language Pathology in the state of North Carolina with current license. Additionally, applicants must be able to perform Modified Barium Swallow studies.

Weeknight Opportunity. Incumbent will work three 12-hour night shifts on Mondays through Fridays. Position also includes working every third Saturday and Sunday and participating in our holiday rotation schedule. Work schedule is 7:00 p.m. to 7:00 a.m.
The Registered Nurse is responsible and accountable for direct and indirect nursing care of patients. She/he is also responsible for the direction and supervision of personnel assigned to her/him.
Minimum qualifications include: Graduate of an accredited School of Nursing. Current North Carolina Registered Nursing license. Applicants should possess BLS certification. Additionally, applicants must have a minimum of one (1) year of previous professional Registered Nursing experience in an acute care setting. Weight Management Coordinator. Lexington Surgical Associates. Full-time Position. Routine work hours are 8:00 a.m. to 5:00 p.m. on Mondays through Fridays. However, our incumbent should be flexible to accommodate working some evening and weekend hours if business needs necessitate.
The Weight Management Coordinator assists with the planning, implementation, and coordination of all activities related to patient management within the Weight Management Center. The Coordinator navigates the patient along the continuum from the ambulatory setting, through the surgical experience, to the follow-up treatment setting. The Coordinator provides education and psychosocial support to patients as they recover and maintain a healthy lifestyle.
Minimum qualifications include: BA/BS in Nursing or Nutrition from an accredited college or university. Dietitian must have completed an approved route to registration by the American Dietetic Association. Current licensure to practice as a Registered Nurse in the state of North Carolina or active registration by the American Dietetic Association and licensure by the N.C. Board of Dietetics/Nutrition. Additionally, applicants must have a minimum of two (2) years experience in obesity disease management as a Registered Nurse or Registered Dietitian in an inpatient or outpatient setting.

The Nurse Aide 1 is responsible for providing care to patients, which is supportive to the care given by Registered Nurses.

Completion of a certified Nurse Aide 1 course and is currently listed with the Division of Facility Services with no substantiated findings of abuse, neglect, or misappropriations of property. Physical Therapist. Rehabilitation Services. Part-time Position. Around 6 hours per week. Incumbent will work a 3-hour shift on Saturdays and Sundays. Work schedule is 8:30 a.m. to 11:30 a.m. on Saturday and Sunday.
The Licensed Physical Therapist is responsible for evaluation and/or treatment of any person when so ordered by a Physician, Dentist or Podiatrist. Treatment is provided through the employment of physical means such as therapeutic exercise, heat, water, electricity, massage, ultrasound and the use of rehabilitative procedures and devices. A Licensed Physical Therapist is responsible for the performance of specialized tests, such as manual muscle testing, goniometry and teaching physical therapy procedures to the patient, family and other health personnel.
Minimum qualifications include: graduate of an accredited curriculum in Physical Therapy, B.S. or M.A. (accreditation is granted by AMA Subcommittee on Professional Education and APTA). Licensed to practice Physical Therapy in the state of North Carolina with current licensure. CPR certification (every 2 years).
Diabetes and Community Education Coordinator

Lexington Memorial Hospital is seeking a full-time Diabetes and Community Education Coordinator to augment its Education staff. Primary position hours are 8:00 a.m. to 5:00 p.m. on Mondays through Fridays; although, applicants must be flexible to work some evening and weekend hours.
Our Diabetes and Community Education Coordinator performs multiple roles: working with diabetic patients in the in-patient and out-patient setting to coordinate care; assessing the community for health education needs and developing, facilitating and implementing programs to meet these needs; and serving in an outreach and community leadership role on health issues.
Minimum qualifications include: Bachelor of Science Degree in Nursing is required, Master's Degree is preferred; Certified Diabetes Educator Certificate (CDE) is preferred or willingness to complete within 18 months of start date; three to five years of previous professional clinical experience encompassing patient care and clinical education; demonstrated ability in adult teaching, demonstrated leadership skills in program planning and development and superior presentation and communication skills.
